D2Type aims to empower people with prediabetes or Type 2 Diabetes to prevent, delay and reverse Diabetes by making small changes resulting in significant health impacts. We believe that everyone who is at risk of Diabetes (Prediabetes) or already has Diabetes deserves to obtain the best care and healthiest life. We aim to partner with employers, health plans and the health system to provide members with personalized tools, resources, and support to achieve a healthier lifestyle and promote lifelong personal health and wellness goals.
